Not sure what you use and your site is certainly more popular that the ones I deal with but I find the following plug ins catch a lot more than anything else I’ve tried.
AVH First Defense Against Spam
Better Moderation
Invisible Captcha
NoSpamNX
About the only thing they catch by mistake are ping backs to other blogs.
